You can now manage your appointments online with 'Patient Access'!
To sign up, simply visit www.patient.co.uk and click the Red 'Patient' tab on the top menu bar. From there, click 'register now' and then 'create a new account'.
Register by finding Chartfield Surgery (our postcode is SW15 6HG) and then complete all your personal details and security questions.
Please write down you User ID and remember you password.
Once registered, you can make, check and cancel appointments online, through the patient.co.uk website, as well as request your repeat medication.
If you encounter problems, please contact the surgery. We are in the process of creating a sign up guide and video which will give step-by-step instructions for patients having difficulty signing up.
