All patients are very welcome to attend our patient group meetings.
CHARTFIELD PATIENT GROUP MEETING – THURSDAY 25th APRIL – 6.30 PM TO 8.00 PM AT THE CHARTFIELD SURGERY.
We very much hope that you can join us this Thursday for what promises to be an interesting and informative meeting. We hope people will come and join in the discussion that we will be having with our local ‘experts’ and the practice.

About the PPG
For patients, run by patients.
- The Chartfield Patient Group is for patients of Chartfield Surgery
- We exist to encourage the voice of ALL patients of Chartfield Surgery
- We are committed to working with the staff at the surgery
- We aim to benefit ALL patients, to influence the delivery of health care services in the surgery and in the local NHS.
The Chartfield Patient Participation Group (PPG) has just 'restarted' and we are keen to involve new members - that is anyone who is a patient at our practice. We'd like you to come along and find out more about the health services in the surgery and give us your views on these services, as well as other NHS services (including hospital/other specialist/community services etc).
If you wish to register your interest (you don't need to commit to do anything!), then please complete this form and we can provide you with further information including the dates of future meetings.
It would be helpful if you could let us know if you are coming to events - so we have an idea of the numbers who are attending.

Wednesday 2nd February - 6pm to 7.30pm.
The A to Z of Chartfield Surgery with a doctor, nurse and practice manager - questions and answers - followed by a 'business' meeting to decide what we do next.
Wednesday 13th April - 6pm to 7.30pm - Presentation and Q&A session on dementia and falls, as well as information on local services for dementia and falls. Click here to see poster.
