You can request your medication through Patient Access, through the below Medication Request Form, or by emailing us at chartfield.admin@nhs.net. Please note that all prescription requests take 2-3 days to complete. Please note that due to the volume of requests we receive, we are unable to notify patients individually when their prescription has been issued. If your prescription is not at the pharmacy within 3 days, please let us know.
