If you don't already have a Patient Access account, please follow the steps below to sign up. Please also visit this link to view instructions and additional information on the Patient Access website.

  1. If you have a compatible smartphone or tablet, first download the app, available through the Apple Store or on Google Play. If you don't have access to a smartphone or tablet, continue to Step 2.

  2. Register for an account on the Patient Access site. 

  3. Once you have set up an account, please send one form of photographic ID to chartfield.admin@nhs.net. Before we can set up your account, we need to verify your identity - our clinical IT system will not allow us to activate your account without proof of ID.

  4. Once we have received your ID verification, we will then send you the Linkage Key and Account ID documents which you will need to set up an account. You can then use these to complete the setup on your account.

ONLINE ACCESS

You can access your appointments, prescriptions, and more at any time by registering for Patient Access. Patient Access is a web-based platform which you can use to order prescriptions, view your medical history, see your past appointments, and more.
